Our Transitional Outpatient Therapy is designed to provide compassionate, structured support to individuals stepping down from inpatient psychiatric care, long-term hospitalizations, or residential treatment facilities. This critical phase of recovery requires specialized attention, and Premier is committed to offering a safe, therapeutic environment that bridges the gap between intensive treatment and community reintegration.

At the core of our program is a multidisciplinary team lead by highly trained mental health therapists with extensive experience in crisis intervention and managing complex emotional and behavioral challenges. We understand that this transition can be marked by emotional instability, uncertainty, and vulnerability. Our clinicians are equipped to provide individualized care that emphasizes stability, safety, and skill-building.

Our Services Include:
 

  • Intensive Outpatient Therapy (Individual, Family, and Group)

    • ​Individual Therapy
      One-on-one sessions focused on managing symptoms, processing recent hospitalization, building coping skills, and developing strategies for long-term wellness.

    • Family Therapy
      Therapeutic support to help families understand mental health challenges, improve communication, and rebuild trust during the transition period.

    • Group Therapy
      Facilitated groups offering peer support, skill-building, and shared processing in a structured and therapeutic setting. Topics may include emotion regulation, relapse prevention, and interpersonal effectiveness.
       

  • Safety Planning and Relapse Prevention
    Proactive planning to manage risk factors and identify early warning signs, with concrete tools to maintain safety and avoid setbacks.
     

  • Life Skills Training and Community Reintegration Support
    Practical support with managing daily responsibilities such as scheduling, hygiene, budgeting, and social interaction—key skills for independence and success outside of a structured setting.
     

  • Coordination with Inpatient Teams, Case Managers, and Community Providers
    Close collaboration with previous treatment providers, case managers, and outpatient resources to ensure continuity of care and seamless transition planning.

Premier Counseling aims to connect clients with a variety of funding sources, including major commercial insurances, state-issued insurances, various Managed Care Organizations (MCO's), and Medicaid.
